The most important muscle which decides the range of movement after a total shoulder replacement is the rotator cuff group of muscles.
In patients with deficient rotator cuff we do a shoulder replacement with a specialized implant called reverse shoulder. This prosthesis is called reverse shoulder because unlike the normal shoulder replacement the spherical part of the prosthesis is attached on the glenoid or the scapular bone. The socket part of the prosthesis is attached on the proximal humerus. Thus the name reverse shoulder prosthesis.
In elderly patients with badly comminuted fractures and patients with rotator cuff arthropathy are the candidates for reverse shoulder replacement. Patients with previous failed fixation of proximal humerus can also get benefit of reverse shoulder replacement
